Carotid Artery Screening

Ultrasound evaluation of the carotid arteries that screens for buildup of fatty plaque — one of the leading causes of strokes.

Atrial Fibrillation Screening

A quick and easy test using a 6-lead EKG designed to identify Atrial Fibrillation. Electrodes are placed on the arms and legs. Atrial fibrillation increases the risk of stroke by 5 times.

Abdominal Aortic Aneurysm Screening

Ultrasound is used to screen for the existence of an aneurysm in the abdominal aorta that could lead to a ruptured aortic artery.

Carotid Artery Screening

Peripheral Arterial Disease Screening

Uses ultrasound and blood pressure measurements to check for peripheral arterial disease (plaque build-up) in the lower extremities.

These terms and conditions ("Terms") govern the use of any voucher ("Voucher") issued by Life Line to a customer ("Customer") identified in a Screening Voucher Order Form ("Order Form"). These Terms supplement the Life Line User Agreement and Terms of Service ("Service Terms") found at https://www.lifelinescreening.com/terms-of-service which shall also govern and apply to the Voucher.

If a conflict exists between these Terms and the Service Terms, these Terms shall take precedence. Customer agrees to convey these Terms to Voucher holders.

1. Use of Vouchers; Process to Redeem: A Voucher may only be used once and only for a screening associated with the Package Description set forth in the Order Form. Depending upon instructions from Customer, Life Line shall either (a) send Vouchers directly to Customer to be distributed to holders, (b) secure holder information from Customer and send Vouchers directly to holders or (c) secure holder information independently and send Vouchers directly to holders.

2. Non-Refundable; No Cash Value: Vouchers are non-refundable and cannot be redeemed for cash, consolidated to a new voucher or replaced after expiration. Vouchers are not legal tender, account cards, credit or debit cards or securities. Unused values will not be attributed to any transactions after the Voucher expiration date.

3. No-Show Forfeitures: If the holder of a Voucher fails to show up for a booked screening, the Voucher shall be forfeited and may not be used for a future screening.

4. Lost/Stolen Vouchers: Vouchers cannot be replaced if lost or stolen. Life Line shall not be responsible for the loss of any Vouchers or any losses or damages suffered by the holder of a Voucher arising from or in connection with the loss or usage of a Voucher.

5. Re-Booking; Cancellation: Except for no-show forfeitures, Vouchers may be re-booked for another screening event so long as the reservation is made prior to the Voucher expiration date. Life Line reserves the right to cancel a Voucher if Life Line suspects fraud or bad faith actions related to the original Voucher.

6. Transfer: Vouchers may not be transferred once the holder has reserved a screening appointment.

7. Privacy Notice: All Vouchers are subject to and governed by Life Line's Privacy Notice found at https://www.lifelinescreening.com/privacy-notice.

8. Limitation of Liability: LIFE LINE'S MAXIMUM AGGREGATE LIABILITY UNDER, ARISING OUT OF OR RELATING TO A VOUCHER SHALL NOT EXCEED THE TOTAL AMOUNT PAID TO LIFE LINE BY CUSTOMER UNDER THE ORDER FORM WHICH INCLUDES THE VOUCHER.

9. Indemnification; Insurance: Each party will indemnify, defend and hold the other party harmless from and against third party actions or written demands directly resulting from the indemnifying party's breach of its obligations hereunder and/or any third party intellectual property rights' breach.

10. Governing law; Venue: The governing law and venue for any dispute arising hereunder will be Ohio.

11. Entire Agreement: The Order Form, these Terms and the Service Terms (including any additional terms expressly incorporated herein by reference) collectively constitute the entire agreement between Life Line and Customer and supersede any previous agreements or representations, either oral or written with respect to the subject matter hereof.
